Audiology and Hearing Conservation

Services:

Provides audiograms for PHAs, flight physicals, separation physical, accession physicals, and Occupational Medicine certification and exposure exams (which includes the Hearing Conservation Program). Diagnostic audiograms are offered as needed.

Hearing tests are available by appointment only.

Scheduling your hearing test:

  • Annual surveillance or certification exams through Occupational Medicine (including those on the Hearing Conservation Program): Appointments are requested through the Safety Department/Officer at the member’s command. The Safety Officer completes and emails the SECNAV 5100/1 form to the Occupational Medicine Scheduling Email group at: usn.whidbey-island.nmrtc-oharbor.mbx.occupational-medicine@health.mil.
  • Flight Physical Audiogram: Service members who receive annual flight physicals do not need to make a separate hearing test appointment. Aviation Prelims will automatically schedule your hearing test on the same day as your flight physical.
  • PHA or Separation Exam: Schedule by calling 1-800-404-4506.

The Audiology department is in Building 2527, Room 1004, across the parking lot from the dental entrance of the main clinic. Turn left when you enter the building and take the half flight of stairs up at the end of the hallway.
